Your database user has failed to connect to the database. Make sure you set the correct host, db name, user and password. And make sure db user has all privilege to the database.

$servername = "localhost";
$username = "username";
$password = "password";
$databassname = "dbname";

$conn = new mysqli($servername, $username, $password, $databasename);

or try this

$db = mysqli_connect('localhost','username','password','databasename');

1. mysqli_connect
2. mysqli_query
3. mysqli_fetch_array
4. mysqli_close

1. make the connection and select the database
2. perform the query on the table
3. print out the data
4. close the connection
