Not understanding why this code is not displaying Form . Help me please. I am Rookie in php

Connection.php - database connection
Header.php - logout

Please check where I went wrong in displaying Form.

When I remove php script from form html got display . But adding php in value results in form to be invisible.

One more thing how to send message to people because when I click on message on sololearn it says email had sent . But I don't receive such email
