How to fetch data selecting radio button in php??

Question

here..
i have 3fields in database table(id,name,mobile_no)

in html..
textbox
radio1=id
radio2=name
radio3=mobile_no

now i insert some values in textbox and select the radio2.. then the data should be fetched according to name..

next time i select the id and insert id then the data should be fetched according to id field..

i hope you under stand my problem..
pleae give me some logics that i solve my problem..
