Do I need a certificate to be a programmer or just keep practicing myself?
hello, I am a registered nurse for 13 years, now I try to learn web development to make my own website. any advice. thank you

For your purpose, a template-based service might be a good idea. Personally, I like WordPress.com, but there are many solutions out there.
A course might be good to learn about digital marketing (seo, sem, etc.)

Well you don't really need a certificate to be called a programmer, all you need is the ability to solve new problem but I do recommend you to get a certificate to serve as an identity that you do know your specialize field of study.

A certificate is only a recognition of your skills. Your website doesn't care if you're a certified developer. Just build it.
But if you don't plan working as a web developer, try to keep things as simple as possible in the beginning. And a simple presentation website can be done even without learning web development, by using online services like Wix and others.
However, learning web development can be really exciting. So if you have the spare time and will to learn, you're likely to have a great time building and improving your website. And there are plenty of tools available, so feel free to try several and choose what works best for you, even if others use something else.

You definitely do not. If you'd like your own site, all you'd need is money to pay for the domain name.
What you do with the site doesn't matter as long as you're not using it to break any laws.
