Web Design/Construction Best Practice

I'm new to this. but say you have a customer who wants a website built. what is best practice on the process from start to finish? I don't have a job in the industry but are you effectively drawing out what you think it should look like? mapping the links on paper? is there software for this design process? or in the real world are there designers who decide how it is going to look and then they pass it to someone who can put that idea into code/reality?

This is the reason some companies have the designer department apart from the programmers. UI/UX is something that can slow down a programmer if he has to do the thinking himself. One of the most increasingly sought for in big tech companies are UX/UI designers. Note: UI differs from UX. The former is a subset of the latter.
