There are a ton of tutorials out there. I would probably start by learning the fundamentals of PHP and MySQL. Once you have that, I'd find an example form (like the one I'm linking here) and work off of that.

It's easiest to learn from something that someone else has started. Use your example form as a base and then start renaming the different fields to ones you would want to use; start testing different data inputs; meddle with different validations requirements; etc.
