Can anyone suggest good book or video for this?

Question

I am facing problems making relational database forms. I mean I made tables in MySQL and connected them with pk/fk. Problem is design in form so, can anyone suggest good video on form creation for that? (Form creation and back-end,not relational database creation), I need good reference material if you can suggest good book on this topic it will also be very useful to me. I am using html, Css, javascript, MySQL, php.

How does that explain relational model? many to many/one to many form creation. I didn't ask for css and html nor for tables. I have tables completed but putting them in front end is tricky. For that I need good references.
