Question concerning formulars in HTML

Hey there
I have a evaluation in which i can give 1 to 4 points (doing this with type=radio) for two questions. After that the average shall be calculated. However my problem is i can only sumbit one formular so the average is wrong.
I hope you kind of get what i want. I need both of them to submit but only can one at a time. How can i create that of "button".
If i know how to post a pic of my code i'll do
