I am quite good in finding out the Output but face problem when given a full program to code ? I am unable to pin point my issue

Most likely it's just the lack of experience which is totally normal. Start coding some really simple programs, such as a percentage calculator, or a unit converter (ie miles to kilometres). Best thing to do is start small and take baby steps, don't try to write overly complicated programs. And most importantly, finish the project! Finishing projects will improve your confidence.

It sounds like you need practice writing the problems rather than solving them. Try what @Daniel said, and write some small simple things to piece code together.
