From a high level perspective, it's no different from the language we converse everyday. We start from standardizing the word choices or we call it tokens/keywords in programming which will serve as the fundamentals.

Similar to natural language, it evolves over time and more features are added to simplify or improve the way of communication. (e.g. brunch anyone? 😉)
