How to integrate an csv in online forms.

Question

Hello i attempted to create an online form with google forms.. but i m blocked as i need to integrate an csv file with numbers nd names of people who will fil the form.. so i have no idea how to do it ... the idea is that i need them to enter their last name and then show them their name and their schol where they work so they can enter the disponibility time for a training.
