How can I make vote counter

Guys, How can I make vote counter I have a vote system and it have "Q: Question & two radio (Yes and no)" Now I want It to show the amount of the user click The Radio (Yes or no)
Example :
🔘Yes - (1 vote)
pls help me in html, css, php and JavaScript

id use a progress bar and when a answer is chosen and submit hit the variable to add 1 to the bar is added. this would be a basic thing

the problem i see is saving the state of the vote progress and identifying an ip so people cant multi vote. these wpuld need form validation using saved information from a server
