Almost each case is different ^^
Anyway, you must prefer to slice your photoshop layout to fit your html structure, against the contrary, code an html structure for a sliced photoshop layout without had considered possibilities/limitations of html/css building :P

Not necessarly: it depends of your stuff, your goal... but you get more probabily to have problem to convert photoshop work to html/css, than to use photoshop for images needs for your html/css design lacks ;)

However, I don't have/use photoshop, and so, don't know what provide exactly as tools for web design: but if I believe your question, all that you can do, seems to not provide any 'export' way ^^ Ad photoshop could be a great picture software, it's will never be a dedicated tool to design html/css layout ^^
