How do we know which subject refers to which color, for e.g. how do we know that #FF0000 refers to red, and #C1250F refers to red mix orange?

By learning hexadecimal and rgb systems and a wide spectrum of color combinations.

(Or just use some kind of color picker as 99.9% of sane people do)
