I need a form with drop down menus with the following description

I'm from Kenya. Kenya has 47 counties but I'll lost a few. Kisumu, Nairobi, Mombasa, Kilifi, Uasin Gishu, Kericho, Bomet. Kisumu county has these sub counties. Muhoroni, Nyando, Kibos, Kisumu East, Kisumu West, Ahero. Muhoroni sub-county has these wards. Muhoroni, Menara, Koru, Koguta. I need a code that restrict choices such that if I were to select Kisumu county, in the next drop down menu, I'll be restricted to sub-counties within Kisumu only and that, downgrades downwards.
