How to create a database, retrieve data and export to excel

Hello

I have just started learning SQL what I was planning to achieve is I have to create a form in html or php to enter data and save into SQL. Later I should have a script that can retrieve the data. It can be whole table or just selected columns. Then after retrieving the data I have to export them into Excel.

Please suggest the setup required. I mean the tools and softwares required to run on Mac.

Any guidance is highly appreciated.
