Do a game designer need to know C#? Is it helpful?

Unity uses C# and JavaScript so if you know one or the other you are good. If you are not using an engine I would not use C#. I personally only use C# for automated tasks in Microsoft Windows. But that is just my opinion and you may be able to write great games in C#. Also, game designers and game developers usually have different roles in the process of making a game.

depends on the game engine.
if you're using Unreal Engine you shouldn't even know how to program, you can create your materials with the Blueprints, and just grab object to the world space.
IDK how materials going in CryEngine and Unity...
