insert new data into database

Question

Sorry for that title if you don't understand. Actually, I need the help to make a registration system using PHP. I use Adobe Dreamweaver CS6/ Micromedia Dreamweaver 8 to build the system.

My question is, how can I create a web page to register the student ID and class. but I want to show the student name in same page. Because I don't want the teacher will be confused which the student he was registered
