Course enroll after screen doubt ??

I have a website in which I made a online course to sell my course but don't know how to make page on which anyone click on buy option then next screen for database will appear for getting user / customer details. I am so confused with that anyone help me to do it plz....

You'd need to look at creating a form, then linking the results to some kind of backend.
