I want to make registration/login form for two different user.can anyone help me?

here is my problem .I am making a webapp for school so there must be two registration one for teachers and another for student can I use same registration form for both or should i make two registration form?? ?

it doesnt matter the database is the same so you decide

but if you want an opinion then mine is that there should be a from for teachers that is showed on the place of the form of students if you click a check box that says "im a teacher" but if you do that you are going to need a second database
