How can I insert data in my data base through MYSQL?

I have a data base, and it is connected to my website, but when I click on submit, somehow, in my database the data appears in 0 in the format of the data type, or don't appears at all. What's happening? Maybe my data types are wrong?

Perhaps the connection of the database is bad, verify that the names of both the database and your tables are the same as you have in your code, check if the connection is fine, when you are making a request prints the database response maybe the problem is in that part of your code
