Basic jobs are usually done by people knowing about both, for example a designer with no coding knowledge can't do web design on his own.
For big project usually designer and programmer are different people, however knowing some coding basics can extremely help him to coaprate with the programmer.

Depends. I honestly think it helps bridge important gaps...and designer/coders can do some neat stuff:
