My code of Autofill Form Field is not working

In my code, I have two input fields. Based on what is selected or written in first input field, second field should automatically filled with matching value. This value will be retrieved from a csv file. However, nothing is happen. Code is not working. I think there is some mistake in code. So, Someone can please help me to figure out that problem...

Missing: input type attribute
Missing: input name attribute
