How to add video/ audio in html?

Question

I made a html for video/audio....1st i add url of the video/audio which is downloaded in my phone and in output it was showing and playing that video or audio but when I open it in chrome, opera or browser it was not showing or playing in none of these apps.... So please tell me how can i add video or audio in html?

Make sure that the video file is in your files for your code, browsers them selves won't read files from your phone.
